Job Description

The Duties of an RN generally include providing routine care, observing resident’s health, assisting doctors and registered nurses and communicating with residents, family members and other caregivers. RN is accountable for ensuring that the utmost in competent care and safety is consistently delivered to all residents.

Required education, training, experience and licenses:

The ideal candidate for this position is a cheerful, compassionate, motivated professional who possesses interpersonal, communication and organizational skills necessary for the job responsibilities of a licensed clinician in a long term care setting.

Must be a graduate of an accredited nursing program with a current license as a Registered Nurse

Prefer at least one year clinical nursing experience, particularly in a long term care setting or with geriatric or other equivalent healthcare experience.

This position reports to the Director of Nursing.

Administrative Functions

Plan, develop, oversee and evaluate the delivery of nursing care and services in accordance with current laws, regulations, guidelines and evidence-based and best practice standards of care applicable to long-term care skilled nursing, memory care assisted living and independent care of seniors. Make written and oral reports/recommendations to the Director of Nursing concerning the operation of the nursing department. Coordinate nursing services with other resident services to ensure continuity of the residents’ total regimen of care and services. Participate in facility on-going survey readiness activities and actual surveys by governing organizations. Keep abreast of regulations and changes in nursing practice and recommend adjustments in care and services that ensure the continued ability to provide the highest quality, resident-centered care and services.. Assist with the review and/or investigation of any incidents, nursing care and service concerns, complaints and grievances made by the resident and/or family, and in collaboration with the Compliance and Quality team as requested. Maintain current knowledge of and adherence to all applicable policies and procedures established by the Board of Directors and management. Personnel Functions

Assist in developing nursing department work assignments and schedules as requested. Provide oversight of nursing personnel, including CMTs and CNAs to ensure that all employees needs are met and that all are performing their work assignments in accordance with acceptable nursing standards of care, applicable evidence-based guidelines and best practices. Ensure that recognition and disciplinary action is administered fairly and without regard to race, color, creed, national origin, age sex, religion, handicap, or marital status. Ensure that parameters of the union contract are adhered to as set in union contract. Assist in developing, scheduling and conducting of timely in-service and just in time training. Assist in delivering effective orientation programs that orient new employees and contract/agency staff to the facility, its policies and procedures, and to his/her job position and duties, including mentoring and precepting as requested. Promote teamwork, mutual respect, excellence, trust and effective communication. Nursing Care Functions

Provide the Director of Nursing with information relative to the nursing needs of the residents and the nursing department's ability to meet those needs. Maintain resident clinical/medical records in compliance with all state and federal guidelines and regulations, including but not limited to the MO Department of Health and Senior Services, the Center for Medicare and Medicaid (CMS) and the Office of the Inspector General (OIG). . Provide oversight of care delivery, including medication passes done by CMTs and assure the quality of care and that care, treatments and medications are being administered as ordered and as scheduled. Monitor residents, participate in developing resident care plans based on individual needs assessed, implement treatments, provide guidance and supervise the work CMTs and CNAs. Takes physician’s verbal orders and execute written orders as necessary and in a timely manner consistent with laws, regulations and standards of nursing practice; channel referrals and orders appropriately through dietary, pharmacy, medical equipment providers, social services, other departments and to the Director of Nursing as needed. Performs physical assessments, evaluate resident’s needs and consults with physicians and others on the care team, including the resident, to assure a current resident centered plan of care, documentation and appropriate care delivery. Provide direct nursing care as necessary

+ Certified Medical Technician (CMT)

General Description: The primary purpose of this job position is to assist with direct care and service delivery, including but not limited to assisting with the administration of prescribed medications to residents and maintaining medical records under the direct supervision of a licensed nurse, taking vital signs and assisting with indirect care and service delivery required in the routine operations of the nursing department, such as compliance and quality audits and other processes related to general operations.

Required education, training, experience and licenses: Excellent verbal communication skills and strong interpersonal skills are essential in this highly collaborative, team-oriented environment and with this position, requiring communication with residents and other members of the care team to deliver excellent care and service within the organization/community. Qualified candidates will have completed a certified Nursing Assistant Training program approved by the MO Department of Health and Senior Services, as well as a Certified Medication Technician program and have experience providing care and services in long term care or equivalent healthcare experience.

+ Certified Nursing Assistant (CNA)

General Description: Assist nurses (RN or LPN) in providing nursing care and activities of daily living and other tasks to support care and services for residents. Reports to Charge Nurse and the Director of Nursing.

Performance Principles: Certified Nursing Assistants are responsible for providing compassionate direct care and services for residents in collaboration with the care team and other service providers, consistent with the Mission and Values of St. Louis Altenheim.

Main Functions:

  • Care for multiple residents in the facility
  • Observe Resident Rights requirements
  • Provide personal care and assistance with activities of daily living and nursing care such as:
    • Assist with personal and dental hygiene including whirlpool, shower or bed bath as preferred by the resident and most appropriate to level of care needed;
    • Shaving
    • Dressing
    • Feeding: Pass and pick-up food trays and meals. Serve nourishments; Assist with feeding; Keep record of intake and output; monitor dining room during mealtimes, as-assigned
  • Assist residents in ambulation, mobility and fall prevention
  • Transfer to and from bed or chair
  • Reposition resident in bed or chair
  • Assist to and from toilet, assist with any toileting needs
  • Provide perineal care (peri-care) as needed to prevent skin breakdown, itching, odor and infection
  • Monitor and record bowel movements
  • Pass and empty bedpans, urinals and emesis basins
  • Collect specimens such as urine, stool, and sputum
  • Provide post mortem care
  • Take and record vital signs, height and weight and notify nurse of any changes
  • Record Activities of Daily Living and care provided on flow sheets
  • Observe residents and note physical condition, attitude, reactions, appetite; report any changes to the nurse
  • Assist nurse with admitting, transferring, and discharging residents
  • Assist nurse in giving treatments and examining resident
  • Assist with restorative and rehab care with proper instruction and supervision
  • Make beds; change linens and assist in keeping resident rooms and common areas free of fall risk and infection prevention and control hazards
  • Use safety devices and standard precautions to prevent the spread of infection and disease such as appropriate handwashing and use of gloves
